Biggest change we can make to the whole model is making it non-profit. Hospitals should be patient first in all instances. That is probably the biggest hurdle to clear and then a bunch of the other issues you mentioned will actually be easier to solve. Right now there is too much private equity involved with insurance, hospitals and medical care in general. The costs associated with medical care are outrageous and also need to be addressed.

Its so easy to say, move. The practical reality is often different. Also why the hell should they move if they aren't being obnoxious but the neighbors are. There's a certain level of decency that one should have in any setting. Being obnoxiously stupid to the point of doing all the things in OP's post makes people like that just fucking unacceptable or intolerable to live with. If people want to live like that then they should go live in the fucking wild. The rest of us actually want to live in civilized society.

I think that's more so they can be prepared to get into character before they go into the simulation. Less to do with the need for the costume.
