Ten seconds on Google brought up the follow up to this story from March 10, 2023.
The department says in a final report today that it has concluded the issue was “isolated” to a single employee who raised assisted dying as an option with four veterans.
Do you think the mistakes of a single employee that the system's check and balances would have caught justifies discrediting the entire system?

It did happen though and that was the fucking point.
One bad person in a system of millions is a bad person, not a failed system. Lol. There are bad people in every profession. Do we get rid of schools because some teachers molest kids? We certainly haven't gotten rid of cops after all that bullshit lol. You can't pick and choose, dude. Bad people are bad people and systems certainly need changing, but you are crying major fucking wolf here.
